Home-based
For clients who are not in need of the residence, structure, and intensity of the therapeutic household Windhorse offers a range of home-based services. The goal is to support a person at home as she or he grows in wellness and independence. A home-based team can support a person and his or family to:
- Make a successful transition to independent living
- Avoid a possible residential placement
- Stay in college locally during periods of difficulty as a student
- Build recovery skills while living at home and being in the community
- Work towards individual life goals that have been challenging, such as finding or keeping employment, making friends, managing affairs, or developing healthy habits.
Home-based teams vary in size and intensity depending on what is needed. Some clients have daily interaction, others once per week or less. A team can be as small as a single clinical mentor, or as large as a full multidisciplinary team. Typically, home-based teams include a team leader, a psychotherapist, and a nurse (with a psychiatrist consulting monthly), and sessions or shifts take place twice or three times per week.
